ATA RAID is Broken on Poorly-Designed Controllers

A poorly-designed ATA RAID controller can have the RAID configuration information overwritten causing the RAID to break. To solve this problem use another brand. You should also complain to the controller manufacturer that the RAID configuration data should be accessible only to the RAID controller firmware itself by mapping out a sector or (more conveniently) one track/cylinder for its own use.
